“KIT Enrollment” Form

You are invited to use this form to enroll your child in the Keep-In-Touch ministry.

Please complete the required fields and press the “Submit” button, at the bottom of the form. You will then receive a message that confirms successful submission of your enrollment.

Your child will then be included in our “Keep-In-Touch” Ministry.  We’ll be emailing notes of encouragement with Gift Cards in the months of:

November
February
April
