About This Form

You may use this form to submit a request to reserve the gymnasium for your group’s event or gathering. Please complete the required fields and then press the “Submit Your Request,” button, that is located at the bottom of the form. You will then receive a message that confirms the successful submission of your request. The church office will contact you shortly, to let you know if your request has been approved for your group’s use.
