About the “Confirmation Enrollment” Form

You may use this form to enroll your child into our 2023-2024 Confirmation class. Please complete the required fields and press “Submit Your Request” at the bottom of the form. You will then receive a message that confirms successful submission of your request. The church office will contact you shortly with more information.

Following submission, you will be emailed our Medical and Photographic Release Form. This is a legally binding form that must be filled-out, electronically signed and then submitted to complete your Confirmation registration. If you are unable to find locate this email, please check your email’s Spam or Junk folder. If you still can’t find it, please contact Pastor Donald or the Church office.
