About Our Enrollment Form

You may use this form to register your child (or children) in our 2016 Discovery Camp. Upon completion and submission of the form, you will be sent automatically to PayPal for payment, which accepts either PayPal or any major Credit Card. For each child enrolled, you will receive a separate email that contains a link to our “Waiver and Medical Release” form. This new process allows you to read, legally sign and return the document to us, all electronically. Upon completion of the enrollment process, you will receive a message that confirms successful enrollment. Discovery Camp Costs

The cost of Discovery Camp is determined by several choices that are offered to you, including:
  • The age of your child (preschool versus grade school)
  • Your selection of our half-day or full-day program
  • The number of days per week your child will be attending (2, 3 or 5 days per week)
  • The number of “Before Care” (7:00-9:00) days needed
  • The number of “After Care” (4:00-6:00) days needed.
  • Your “By the week” or “Full Summer” pay-ahead choice
  • Attendance during the week of ABC/VBS

There are NO refunds for late
attendance or absences

Please note that additional costs for field trips are required for Wednesday attendance.

Palatine Pool passes or Punch Cards are required if attending on Pool Days.

Enrollment / Payment Instructions

All fields containing a red asterisk are required. When you have completed the form has been completed and your press the “Submit” button, you will be taken to our payment page. Payment may be made with Paypal or any major credit card. To make the payment with a Visa, Master Charge or an American Express credit card, click on the “Make payment with…” link.

Completing Your Enrollment

Following payment and completion of all required fields on the PayPal payments page, click on the “Continue” button, located at the very bottom. If you have questions or difficulties, please contact the ESS Director at (224) 801-8343 or send an email to dcamp@ilcp.org.
For each child registered, be sure to open the email containing a link to our Waiver and Medical Release form. Complete the open fields and electronically sign it, It will then be automatically returned to us as a legal document.

Important Note
: Please add the following two entries to your Contact List, so that you correctly receive important emails from us regarding your enrollment(s). Otherwise, your email program may treat our follow-up email(s) as junk or spam and relegate them to your email’s junk or spam folder.
