About the Christmas Musical DVD Request Form

Please use this form to request DVD(s) of our Christmas Musical “Love Was Born a King”  We also have a limited number of our 2019 Christmas Musical DVD that may also be requested.

An optional donation can be made electronically on-line, or at the church office. Paypal and all major credit cards are welcome. The recomended donation guidance is $2.00 per DVD requested and $3.00 per shipping destination (if used), but this donation is totally optional. You are encouraged to share these wonderfull DVDs with your friends, relatives and coworkers, as a method of sharing the Gospel and spreading God’s word into the world! The ability to optional send DVDs to your friends and family members is also supported on this form and you may optionally include a personal note to your fiend or family member!

Please complete all required fields and press “Submit Your Request” at the bottom of the form.   If you have elected to make a donation, you will be directed to PayPal, where you may use your PayPal account or any major credit card. After that, you will then receive a message that confirms successful submission of your request.
