About the
“Confirmation Enrollment” Form

Please use this form to enroll your child in our 2020-21 Confirmation Class. Complete all required fields and press the “Submit” button that is located at the bottom of the form. You will then be taken to our secure PayPal payment processor, where you can pay for the class, resources and various options, using either PayPal, Visa, Master Charge or American Express. Your enrollment is not considered complete until your payment has been accepted and processed. There are no refunds.

A Note about Finances

If you are are only able to make a partial payment or unable to make a payment at this time, please contact Pastor Donald (224-801-8347).  He will help you register offline.
